The BWFA Longevity Questionnaire
WELCOME TO THE BWFA BLUE ZONE LONGEVITY ASSESSMENT
At BWFA, we believe that the best financial plans begin with a deep understanding of your whole life, not just your money, but your health, lifestyle, relationships, and values. That’s because how long you live, how well you live, and how connected you feel to your purpose all directly impact how you should plan for the future.
To support this holistic approach, we’ve developed the BWFA Blue Zone Longevity Assessment, a brief yet powerful tool designed to give us a clearer picture of your lifestyle today so we can help you build a more thoughtful, sustainable financial future.
This assessment is inspired by the Blue Zones, regions around the world where people live the longest, healthiest, and most meaningful lives. These communities, found in places like Japan, Italy, Greece, Costa Rica, and California, share a number of common lifestyle habits: regular movement, plant-based diets, strong social bonds, low stress levels, and a profound sense of purpose.
Our goal is to create a Blue Zone around the BWFA community, helping our clients not only live longer, but live better, with the financial confidence and life structure to support healthy, purpose-filled retirement years.
WHAT IS THE BWFA BLUE ZONE LONGEVITY ASSESSMENT
This 26-question lifestyle assessment takes just 5–10 minutes and helps us understand the habits that may impact your healthspan and longevity. Your answers allow us to align your financial plan with your real-life needs—whether that means planning for a longer retirement, healthcare costs, or daily quality of life. The questions cover six key areas: basic health and demographics, diet and nutrition, physical activity, stress and mental health, social connection and purpose, and preventive care and family history.
